Enhanced Services Case Manager (Personal Services)



GENERAL FUNCTION: This position provides enhanced case management for parenting or pregnant participants, and participants struggling with substance abuse and/or behavioral health symptoms. Will provide information, advocacy, and collaborative treatment planning within Casa de Amparo and in cooperation with other agencies. This position will have a caseload of 10-13 participants. This position can provide individual and group therapy to participants based on qualifications.

MAJOR TASKS:

  1. Provide appropriate enhanced case management and/or therapeutic services for assigned clients weekly.
  2. Maintain a caseload of 10-13 participants.
  3. Assist in maintaining all charts and documentation including but not limited to: Community Care Licensing forms, documentation of services, contact sheets, and transition dispositions.
  4. Connect participant to appropriate services based on client referral and/or may provide individual, family therapy, parent education and substance abuse education, as approved and appropriate.
  5. Assist in assessing participant's needs, strengths, level of functioning and assist in establishing goals. Completion of client treatment plan and diagnosis depending on experience and qualifications.
  6. Provide trauma-informed treatment services that are culturally responsive, demonstrating knowledge of and respect for the diverse cultural backgrounds and norms of individuals. Services may be delivered in individual or group settings, as appropriate.
  7. Participate in Child and Family Team meetings.
  8. Consult with designated supervisor regarding assessment and treatment of clients.
  9. Consult with collaborative partners in the best interest of each client, as necessary and appropriate (i.e. County Social Workers, health care professionals, school staff, Regional Center service providers, etc.).
  10. Consult and communicate with the client's natural support system, as appropriate.
  11. Participate in weekly clinical individual and/or group supervision, as appropriate.
  12. Attend all staff meetings and in-service trainings as required.
  13. Assist with crisis assessment and behavioral interventions, as appropriate.
  14. Transport clients to appointments using agency and/or personal vehicle, as needed.
  15. Perform other tasks as assigned.
  16. Ability to comply with a flexible schedule.

QUALIFICATIONS:

  • Must be able to be associated with Casa de Amparo through Community Care Licensing (CCL), which typically includes clearance by FBI, DOJ and CACI.
  • Minimum of 2 years' experience in a public or private child welfare social services setting or specific experience working with transition age youth who are 16 to 24 years of age, preferred.
  • Qualify as Enhanced Services provider as at least one of the following:
    • Graduate of an accredited university with a master's degree in behavioral science/arts preferred.
    • A California Registered associate or licensed as an LMFT, LCSW or LPCC with active BBS number required to practice therapy.
    • Lived experience relating to former foster youth, for example, being a former foster youth, system impacted (juvenile hall or child welfare) an adoptee, survivor of child abuse/neglect or similar experiences. Lived Experience can also be identified as being a parent and/or having lived experience with mental health and/or substance abuse.
    • Experience providing treatment to individuals who are struggling with substance abuse and/or mental health symptoms, and/or parenting/pregnant individuals.
  • Knowledge of developmental needs of children ages 0-18 years throughout the developmental stages.
  • Must possess strong interpersonal, time-management, organizational, written and verbal communication skills, and intermediate computer literacy. English proficient. Must have ambulatory, auditory and visual skills to observe participants and staff.   
  • Must have the physical ability to participate in activities and perform related physical skills, including, but not limited to, crouching, lifting 50 pounds, crawling on hands and knees, and walking up and down stairs. 
  • Ability to comply with all employment requirements including, but not limited to CPR/Pediatric 1st Aid, physical, TB and drug screen clearance, clear DMV record and approval from insurance carrier to drive agency vehicles. Must provide proof of immunization against influenza, pertussis, and measles.
  • Must be at least 21 years of age.
  • Bilingual English/Spanish preferred.
