REGISTERED NURSE HOSPICE CASE MANAGER
SUPERVISION RECEIVED: COMMUNITY HOSPICE NURSING SUPERVISOR
SUPERVISION GIVEN: HOME HEALTH AIDES, LPNS
GENERAL STATEMENT: Assumes responsibility for overall management of hospice
patients/families/caregivers. Along with the interdisciplinary group, assesses plans and
formulates and individualized nursing care plan and comprehensive services to patients/
families/caregivers. Participates in on call rotation every 10 weeks fielding calls for hospice
patients after hours.
