Resident Wellness Coach (Healthcare)



Essential Duties and Responsibilities
  • Oversees planning, development, and delivery of resident health and wellness programs. Evaluates effectiveness of programs and makes adjustments as needed
  • Ensures that schedules for programs and activities are planned and communicated in a timely an efficient manner.
  • Ensures team members develop ongoing relationships with residents to increase retention and participate in programing and activities.
  • Works to ensure a risk-free environment by promoting safe practices and conditions. Reports accident and injuries and completes resident incident reports.
  • Partners with health services, residents, and family members to create a person-centered care plan specific to the residents needs and wellness goals.
  • Conducts one on one personalized training to assist residents in improving and maintaining their physical wellbeing.
  • Oversees tracking and reporting of resident health and progress.
  • Develops and implements goals and outcome measurements to establish effectiveness of Sullivan Park's Wellness program.
  • Partners with all community departments to ensure residents needs are met in a holistic manner. Maintains a high level of communication between departments.
  • Maintains professional knowledge of industry trends and best practices through continuing professional education, maintaining personal network, and participating in professional organizations.
  • Performs other duties as assigned.

Qualifications and Experience

Three (3) years of experience in fitness industry/allied health profession, health management, strength and conditioning, and/or personal/athletic training required. Ability to empower and encourage residents. Experience in senior living and working with disabled individuals preferred. Certification as a Personal Trainer required (ACE, NASM, PTAC, ISSA, ACSM, or NSCA certifications).

Education and/or Experience

Bachelors' degree in exercise science, recreation management, kinesiology, personal training, or similar health field of study preferred, or equivalent, knowledge and skills obtained through a combination of education, training, and experience.

Language Skills

Ability to read technical procedures.

Ability to read and comprehend policy and procedure manuals.

Ability to effectively present information and respond to questions from managers and employees.

Mathematical Skills

Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.

Reasoning Ability

Ability to solve practical problems.

Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Certificates, Licenses, Registrations

Certification as a Personal Trainer required (ACE, PTAC, ISSA, ACSM, OR NSCA)

Physical Demands

The essential functions of this position require the following physical abilities: Standing and /or walking very frequently. Sitting occasionally. Reaching with hands and arms frequently, pushing/pulling very frequently. Talking and /or hearing very frequently. Tasting and /or smelling very frequently. Lifting up to 50 pounds frequently. Climbing, balancing, stooping, kneeling, crouching or crawling occasionally. Close, distance, color, peripheral, and depth perception in vision: ability to adjust focus.
