What You'll Do:

• Deliver personalized therapy treatments to short-term rehab and long-term care residents
• Collaborate with our interdisciplinary team to optimize patient outcomes
• Document progress and adjust care plans as needed
• Educate residents and families on continued care strategies

What We're Looking For:

• Valid California Physical Therapist license
• Experience in skilled nursing or post-acute care preferred
• Strong communication and documentation skills
• Team player with a resident-first mindset
