Peninsula Post Acute is seeking a dedicated and compassionate Social Services Assistant to join our team in a fast-paced skilled nursing environment. Responsibilities:
Collaborate with residents and their families to establish goals and develop a course of action that promotes self-care, independence, and overall well-being.
Provide medically related social services to support the highest practicable physical, mental, and psychosocial well-being of each resident.
Assess social, family, psychological, and emotional needs to develop comprehensive care plans addressing social services issues.
Offer emotional support and guidance to residents and families facing challenges such as coping with physical disabilities, health-related fears, and grief associated with loss or death.
Actively participate in discharge planning and coordinate with appropriate community agencies and referrals as needed.
Requirements:
High school diploma or equivalent required.
Experience in skilled nursing and familiarity with PointClickCare (PCC) preferred, but not required.
Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
Excellent communication and interpersonal skills, fostering positive relationships with residents, families, and staff.
Ability to evaluate social and family information, psychological, and emotional needs to assist in assessing social services needs as well as develop care plans for social services issues.
